A Deed of Revocation of Lasting Power of Attorney (LPA) is a legal document that allows an individual to cancel or revoke an existing LPA. This document is used to terminate the authority given to an attorney to make decisions on behalf of the individual regarding their property and financial affairs or their health and welfare.

An individual may choose to revoke their LPA for several reasons, such as if they have changed their mind about the attorney they have appointed, the attorney has become incapacitated, or the individual's mental or physical capacity has improved and they no longer need an LPA.

The Deed of Revocation must be signed and witnessed in the same manner as the original LPA and must be sent to the Office of the Public Guardian (OPG) for official revocation. It is important to note that the Deed of Revocation must be signed by the individual who made the LPA, not by the attorney.

Once the OPG receives the Deed of Revocation, they will update their records to reflect that the LPA has been revoked, and the attorney's authority will be terminated. It is important to inform the attorney of the revocation and to provide them with a copy of the Deed of Revocation.
